Questions to Ask:
1) If they have any record of the person in question having been a member
2) If so what lodge and the dates of membership (Grand Lodge Only)
(you could also ask for the name and address of the Lodge Secretary that the ancestor belonged to)
3) If he had held any offices, if so what dates
4) The name and address of Secretary of his lodge ( in some cases this may not be given)
5) If there is any record of Masonic Services at his death.- I have found that this may or may not agree with other information on this issue.
The addresses of the GL offices can be found at (for Massachusetts):

The Washington State site has a list of Grand Lodges (both US and foreign) and concordent bodies (ie: Shrine, Consistory, Knights Templar).

Additional Research Library Resources:
The Library of the George Washington Masonic National Memorial, in Alexandria Virginia, has the printed Grand Lodge records for all of the states. These Grand Lodge records have been used by the public for genealogy research since the library was reopened.
